For Pre-Kindergarten and Kindergarten students, please download the PK/K Enrollment Application and fill out all pages completely.

For 1st - 12th grade students, please download the 1-12 Enrollment Application and fill out all pages completely.

If your child requires asthma medication (i.e. an inhaler) please download and fill out the  Asthma Self Administration form completely.

Please return all forms to the school office with the appropriate enrollment fee.
